Referrals & Placement Information
 

Chrishaven emphasizes the importance of frank dialogue with potential residents, parents, social service agencies, medical providers and vocational/school personnel. The personal goals of each resident are clearly defined from the first day of admission.  The resident‰Ûªs needs are evaluated and service plans are initiated to meet individual needs and personal goals.  Residents programs are established and modified on an ongoing basis relative to the individuals growth and opportunities for more independence and less structure will be introduced as deemed appropriate.
 

Referrals from a wide variety of sources initiate the admission procedure.  Referral packets should include:

  • Medical & Social History
  • Current Program
  • Psychometric testing (when available)
  • Psychological and/or Psychiatric Reports
  • Recent Physical Exam
  • Medication History
  • Neurological Exam When Appropriate
  • Description & Frequency of Current Behaviors and Emotional Problems


Admission Candidates are evaluated by the Director, Social Worker, House Coordinators, Client Coordinators and others, as deemed appropriate.  Referral materials are used to establish programming, and often a personal visit is scheduled prior to placement to aid in the transition process.
